I asked Steve to write the operational foreword to Diving Crew Resource Management because he is one of the few people who can speak with authority to both the audit standard and the dive deck. Three decades in commercial and defence diving, Chartered Quality Professional and Lead Auditor, working across ISO 9001, ISO 45001, the Diving at Work Regulations and MOD, IMCA and ADC frameworks — he holds the technical authority and the systems view at the same time. His judgement that incidents rarely stem from a single technical failure, but from breakdowns in coordination and shared understanding, matches what the evidence consistently shows, and it is why his audits surface gaps before they become incidents. He does not soften findings to keep a client comfortable, and he does not consult from theory. When assurance has to stand up to a regulator, a client or a board, Steve is who I would call.
